	The artist, title, and catalog number for each item you want .
	A valid Visa, MasterCard or American Express number and expiration date.
	Your name, billing address and shipping address, e-mail and phone number.
	US shipping: $6 for first video, book or 3 audio tapes, $3 each for additional pieces Canada shipping: $10 for video, book or 3 audio tapes, $5 each for additional pieces International shipping: $12 for video, book or 3 audio tapes, $5 each for additional pieces Note: On large orders, appropriate shipping costs will be calculated based on weight. PAL video conversions: $10 each conversion (please specify if you want PAL)
	California residents will be charged applicable sales tax
